You’ll be matched with a dedicated Care Manager who will organise any allied health appointments and create a care visit schedule that works best for you. As your needs change over time, your Care Manager will be right there with you, making sure you are consulted and informed at every step.
Care Plan Managers are experienced, thoughtful, and understand the services available in your local area. They know how things work, how much they should cost, and which services will be most beneficial for you.

How Care Management Works
You’ll be assigned a designated Care Plan Manager, who will take over the administration and organisation of your care, so you don’t have to worry about it.
Your Care Plan Manager will organise your appointments, services, home modifications and equipment, and anything else you need for your care to run smoothly.

For this reason, suitability for self-management is considered on an individual basis and may depend on factors such as support needs, clinical considerations, and the ability to meet quality and safeguarding requirements.
