About Palliative Care at Home
Palliative care at home provides compassionate, specialised support for people living with life-limiting conditions. It focuses on comfort, symptom management, and quality of life, in the familiar surroundings of home.
Our palliative-trained caregivers work alongside your medical team to deliver holistic, coordinated care that upholds dignity and supports the whole family throughout the journey. And because every situation is different, we offer flexible care options, including hourly support, live-in care, and 24-hour care at home, so your care can grow with you.
The End-of-Life Pathway
The End-of-Life Pathway is a government-funded option under the Support at Home program, designed to support people who wish to remain at home during their final months.
What it is: The pathway provides up to $25,000 in funding over a 12-week period for people assessed as having three months or less to live. It is available to Australians aged 65 and over (or 50 and over for Aboriginal and Torres Strait Islander people).
What it covers: Funding can be used for a range of services tailored to individual needs, including increased personal care and nursing support, allied health services such as physiotherapy and occupational therapy, household tasks, and emotional and practical support for the person and their family.
How it works: Eligibility is assessed by a doctor or nurse practitioner. The pathway is open to both new participants and those already receiving Support at Home services. Once approved, Dovida’s specially trained caregivers work alongside your medical team to deliver integrated, coordinated care.
Dovida’s role: As an approved Support at Home provider, Dovida delivers services under the End-of-Life Pathway with caregivers who have specialised palliative care training. We help families navigate the process, coordinate services, and ensure the care plan reflects the person’s wishes, values, and goals.

What is the End-of-Life Pathway?
