Understanding the True Cost of Home Care: What to Ask Before You Choose
Choosing a Support at Home provider is one of the most important decisions you’ll make for yourself or a loved one. While comparing hourly rates seems straightforward, the reality is more complex. Understanding the true cost of home care requires looking beyond the headline figure to see what you’re actually paying for.
Why Total Cost Matters
Your Support at Home budget is precious. Every dollar should go towards care and services that improve quality of life. Yet it can be confusing navigating the different pricing models of providers – transport fees, different service costs, or cancellation charges that weren’t clearly explained upfront.

Your Rights: Support at Home Costs
Under the Support at Home program, providers can charge for:
- Different rates for services on the weekends, public holidays or in the evening
- Cancelled visits if 24-48 hours’ notice is not given (check the cancellation period with your chosen provider)
- Kilometres travelled during the visit
Providers cannot charge for the following:
- Onboarding or entry fees – this is included as part of the 10% care management fee taken from the quarterly budget
- Exit fees when you leave
- Admin fees to cover the costs of producing statements, recruiting caregivers etc
- Package management fees: this existed with Home Care Packages (HCPs), but should be included in the service price for Support at Home.
All costs to deliver the service must be included in the unit price.
Provider obligations to ensure pricing transparency:
- Pricing must be published on both their website and the My Aged Care website.
- The agreed price of services must be documented in the participant’s service agreement and services cannot be charged more than the amount agreed
- Any pricing changes must be communicated to the participant with a new service agreement signed.
The Hidden Components: What to Look For
Transport Costs
Many older people receive transport services to help them get to and from appointments, to the shops or even a social outing. However, providers may charge for this in different ways: some providers include a specific amount of kilometers that can be travelled within their hourly rate, whilst others charge separately for this on top of the care hours. It’s important that you understand:
- Whether this is a single or round trip
- What kilometers are covered and what are the additional charges if the journey is longer
Example scenario – You book a two-hour morning visit to go to the shops 10km away:
- Provider A charges their hourly rate ($120 per hour) for two hours of care with the 20km round-trip included in the hourly rate, costing a total of $240.
- Provider B charges their hourly rate ($120 for care) for the same two hours of care, plus they charge $45 for travel of up to 50km. This brings the total to $285 for the same two-hour visit to the shops.
Provider B is nearly 20% more expensive for the same service.
Minimum Visit Duration:
Some providers require minimum visit lengths (e.g., two or three hours) which may not suit your needs. If you only need 90 minutes of support but must book a three-hour minimum, you’re paying for unused time. You may wish to consider whether shorter visits better suit your needs and routine, or whether longer scheduled visits provide better value and consistency for your circumstances.
Cancellation and Rescheduling Policies
Life happens. Medical appointments run late, you’re unwell, or plans change. Understanding cancellation fees upfront prevents unwelcome surprises. Ask:
- “How much notice do I need to give to avoid cancellation fees?”
- “Can I reschedule without penalty?”
- “What happens in emergencies or if I’m hospitalised?”
Weekend, Evening and Public Holiday Rates
If you need care outside standard weekday hours, rates often increase significantly. Some providers charge 150-200% of standard rates for weekends and public holidays. Make sure you understand these loadings if you’ll need care at these times.
Questions to Ask Every Provider
When comparing providers, use these questions to understand their full service offering:
